1. Consent & Authorization

I, the client named above, hereby grant Bella Gaïa Hydration Head Spa, its owners, employees, contractors, representatives, and assigns the right and permission to take and use photographs, videos, and/or audio recordings of me during my visit, service, or participation in spa-related activities.

I understand that these materials may be used for purposes including but not limited to:

• Marketing and promotions on social media, website, and printed materials

• Training and educational content

• Advertising and public displays

• News or press features related to Bella Gaïa

2. Usage Rights

I agree that Bella Gaïa may use, reproduce, publish, and distribute my images, likeness, voice, or recorded statements in any format (including print, video, digital, or online) without compensation. Before public posting, Bella Gaïa will offer to show me the selected content, as described in Section 3.

I understand that the content may be edited, combined with other media, or modified for creative or promotional use.

3. Acknowledgment & Confidentiality

I understand that no sensitive or private body areas will ever be shown in public media. Before posting, Bella Gaïa staff will offer to show me the photo or video and obtain a second verbal confirmation to ensure I am comfortable with the selected images.

If at any time I wish to revoke permission, I may do so in writing to contact.bellagaia@gmail.com or by texting 713.909.9803, and Bella Gaïa will make reasonable efforts to stop future use of the materials. Revocation applies to future use only; content already published, printed, or shared before my request is received may not be fully removable.

4. Liability Release

I hereby release Bella Gaïa Hydration Head Spa, its owners, employees, contractors, representatives, and affiliates from all claims, demands, and causes of action which I may have arising from the use of my image, voice, or likeness, including but not limited to invasion of privacy, defamation, or any right of publicity claims.

5. Agreement Confirmation

By signing below, I confirm that:

• I am at least 18 years old, or my parent or legal guardian is signing on my behalf below.

• I have read and understood this release form, and it remains in effect unless I revoke it as described in Section 3.

• I voluntarily agree to the use of my photo, video, and/or voice recordings as outlined above and for the uses I selected below.
